Mr. Diabetes® Achievement Award 

In November 2003, in conjunction with Diabetes Awareness Month, the Defeat Diabetes Foundation is announcing the establishment of The Mr. Diabetes® Achievement Award, a recognition program for diabetics and diagnosed pre-diabetics who are contributing to knowledge, awareness and/or prevention of the diabetic condition in the United States.  

The award, which consists of public recognition and a cash prize, will be given to a diabetic or diagnosed pre-diabetic individual or group for work that contributes to the knowledge, awareness and/or prevention of diabetes. The Grand Prize will be $3500, Second Place $1500 and Third Place $500 with recognition being given to the top ten finalists. The 2004 Award will focus on efforts or campaigns creating awareness and steps to prevent the disease. 

Andrew P. Mandell (Mr. Diabetes®) Executive Director of the Defeat Diabetes Foundation is currently undertaking his own epic awareness campaign by walking the 10,000+ mile perimeter of the United States (The Mr. Diabetes® Wake Up and Walk® Tour).  

The Foundation wants to honor individuals with an award for their own efforts to prevent diabetes and create public awareness about the problem. We are interested in hearing about ways that diabetic individuals are helping others become aware of the incidence and dangers of diabetes and helping to prevent diabetes in their own communities.

Peer or self-nominations will be accepted from the public through February 29, 2004. To make a nomination, complete the information below and provide a description of your personal achievements related to diabetes awareness and/or prevention. Each entry must include a 500 to 1,000 word summary (one to two pages, single-spaced, typed) detailing:

1.   What project(s) you have undertaken that have aided in creating awareness about diabetes or preventing diabetes.

2.   Why you think awareness and prevention are important in the effort to fight diabetes.
